class=\"asd-regulation-title\">\n      Regulations <span>organisational<\/span>\n    <\/h1>\n\n    <p class=\"asd-regulation-intro\">\n      The Rules and Regulations set out the organisation of the work of the AS-Dent Studio treatment centre, the rules for the provision of health services and the basic rights and obligations of patients.\n    <\/p>\n\n    <div class=\"asd-regulation-content\">\n\n      <h2>Organisational Regulations of the AS-Dent Studio treatment centre<\/h2>\n\n      <h2>\u00a7 1 General provisions<\/h2>\n      <ol>\n        <li>\n          These regulations define the organization and order of the process of providing health services by the medical entity operating under the name of <strong>AS-Dent Studio<\/strong>, hereinafter referred to as the \u201eTreatment Provider\u201d.\n        <\/li>\n        <li>\n          The treatment provider operates at the address: <strong>Rybacka 32A, 59-800 Luba\u0144<\/strong>.\n        <\/li>\n      <\/ol>\n\n      <h2>\u00a7 2 Scope of the regulations<\/h2>\n      <ol>\n        <li>\n          The Rules and Regulations apply to all persons working or providing services to the Treatment Provider in the course of their treatment activities, regardless of the legal basis of their employment or collaboration.\n        <\/li>\n        <li>\n          The regulations also apply to all patients to whom health services are provided at AS-Dent Studio.\n        <\/li>\n      <\/ol>\n\n      <h2>\u00a7 3 Legal Basis<\/h2>\n      <ol>\n        <li>\n          The Regulations have been prepared on the basis of the applicable legislation, in particular:\n          <ul>\n            <li>Act of 15 April 2011 on therapeutic activity,<\/li>\n            <li>Act of 6 November 2008 on Patients' Rights and Patients' Ombudsman,<\/li>\n            <li>Act of 5 December 1996 on the professions of physician and dentist,<\/li>\n            <li>provisions on the keeping, storage, access and processing of medical records.<\/li>\n          <\/ul>\n        <\/li>\n      <\/ol>\n\n      <h2>\u00a7 4 Purpose and tasks of the Treatment Facility<\/h2>\n      <ol>\n        <li>\n          The primary objective <strong>AS-Dent Studio Sp. z o.o.<\/strong> is the provision of health services by persons with the appropriate qualifications and authorisations under the law.\n        <\/li>\n        <li>\n          The basic tasks of the Treatment Entity include, in particular:\n          <ul>\n            <li>planning, organising and carrying out preventive and therapeutic activities,<\/li>\n            <li>providing individual medical advice and consultation,<\/li>\n            <li>conducting information and education activities in the field of the speciality represented,<\/li>\n            <li>cooperation with other health care units, universities, schools, workplaces, organisations and associations,<\/li>\n            <li>providing specialised services to individuals and entities interested in permanent or periodic care,<\/li>\n            <li>carrying out preventive information and education activities,<\/li>\n            <li>issuing opinions and decisions on the state of health, in accordance with current legislation,<\/li>\n            <li>carrying out other tasks as assigned by the authorised person.<\/li>\n          <\/ul>\n        <\/li>\n      <\/ol>\n\n      <h2>\u00a7 5 Organisational structure<\/h2>\n      <ol>\n        <li>\n          There is a separate treatment facility within the Treatment Entity under the name of <strong>AS-Dent Studio<\/strong>.\n        <\/li>\n        <li>\n          The following organisational units operate within the treatment facility:\n          <ul>\n            <li>Dental outpatient clinic,<\/li>\n            <li>X-ray \/ CT Dental Laboratory.<\/li>\n          <\/ul>\n        <\/li>\n      <\/ol>\n\n      <h2>\u00a7 6 Type and scope of services provided<\/h2>\n      <ol>\n        <li>\n          The treatment provider provides outpatient specialist treatment services including dental treatment.\n        <\/li>\n        <li>\n          The scope of services includes, in particular:\n          <ul>\n            <li>dental prosthetics,<\/li>\n            <li>dental surgery,<\/li>\n            <li>conservative dentistry with endodontics,<\/li>\n            <li>orthodontics,<\/li>\n            <li>paediatric dentistry,<\/li>\n            <li>periodontology.<\/li>\n          <\/ul>\n        <\/li>\n      <\/ol>\n\n      <h2>\u00a7 7 Place of performance<\/h2>\n      <ol>\n        <li>\n          Health services are provided to patients within the Dental Clinic located at:\n          <strong>Rybacka 32A, 59-800 Luba\u0144<\/strong>.\n        <\/li>\n      <\/ol>\n\n      <h2>\u00a7 8 Organisation of admissions and registration of patients<\/h2>\n      <ol>\n        <li>\n          Services within the AS-Dent Studio organisational unit are provided on the following days and times:\n          <ul>\n            <li><strong>Monday:<\/strong> 10:00-18:00<\/li>\n            <li><strong>Tuesday:<\/strong> 10:00-18:00<\/li>\n            <li><strong>Wednesday:<\/strong> 10:00-18:00<\/li>\n            <li><strong>Thursday:<\/strong> 10:00-18:00<\/li>\n            <li><strong>Friday:<\/strong> 10:00-18:00<\/li>\n            <li><strong>Saturday and Sunday:<\/strong> by prior arrangement<\/li>\n          <\/ul>\n        <\/li>\n        <li>\n          Services are provided either on the day of the request or at a time agreed with the patient.\n        <\/li>\n        <li>\n          Patient registration is by application:\n          <ul>\n            <li>made in person or through a third party at the place where services are provided,<\/li>\n            <li>telephone number: <a href=\"tel:+48532008811\">+48 532 00 88 11<\/a>,<\/li>\n            <li>by e-mail at: <a href=\"mailto:rejestracja@asdentstudio.com\">rejestracja@asdentstudio.com<\/a>.<\/li>\n          <\/ul>\n        <\/li>\n        <li>\n          The patient has the right to receive immediate health services in the event of a threat to health or life.\n        <\/li>\n        <li>\n          Patient services are provided by staff with the required qualifications and entitlements set out in separate legislation.\n        <\/li>\n        <li>\n          Health services are provided in accordance with the requirements of current medical knowledge.\n        <\/li>\n        <li>\n          Health services are provided with due care, under conditions that meet professional and sanitary requirements.\n        <\/li>\n        <li>\n          Persons providing health care services shall be guided by the rules of professional ethics laid down by the competent self-governing bodies of the health professions.\n        <\/li>\n        <li>\n          Health services are organised in a way that makes it as convenient as possible for patients to access services.\n        <\/li>\n        <li>\n          Medical certificates concerning the health of patients are issued in accordance with the regulations of the Act on the Profession of Physician and Dentist.\n        <\/li>\n      <\/ol>\n\n      <h2>\u00a7 9 Referrals, diagnostic tests and consultations<\/h2>\n      <ol>\n        <li>\n          Doctors providing healthcare services in the Healthcare Provider may refer patients for in-patient treatment, diagnostic tests or specialist consultations by issuing a referral.\n        <\/li>\n        <li>\n          Referral for consultation, specialist advice or planned hospitalisation should be preceded by diagnostic tests to justify and supplement the referral, in particular orthopantomographic X-rays or craniofacial cone tomography, depending on the medical indication.\n        <\/li>\n      <\/ol>\n\n      <h2>\u00a7 Organisational tasks of units and staff<\/h2>\n      <ol>\n        <li>\n          The main organisational tasks of all organisational units include:\n          <ul>\n            <li>the performance of the tasks of the Treatment Entity and the purpose for which it has been established, as set out in generally applicable laws and these Regulations,<\/li>\n            <li>competent and timely performance of the duties undertaken,<\/li>\n            <li>taking care of the good image of the Treatment Provider,<\/li>\n            <li>ensuring the smooth running of the facility,<\/li>\n            <li>respect professional secrecy, professional ethics and patient rights,<\/li>\n            <li>carrying out other activities as specified in orders, instructions or internal regulations.<\/li>\n          <\/ul>\n        <\/li>\n\n        <li>\n          The main tasks of the AS-Dent Studio organisational unit include:\n          <ul>\n            <li>examining the patient and providing medical advice,<\/li>\n            <li>determining the severity of the patient's health problem,<\/li>\n            <li>determining the direction of treatment,<\/li>\n            <li>making a diagnosis,<\/li>\n            <li>health assessment and opinion,<\/li>\n            <li>providing preventive care,<\/li>\n            <li>carrying out diagnostic activities,<\/li>\n            <li>providing emergency first aid,<\/li>\n            <li>carrying out preventive dental care,<\/li>\n            <li>providing dental prosthetics services,<\/li>\n            <li>providing dental surgery services,<\/li>\n            <li>Providing services in the field of conservative dentistry with endodontics,<\/li>\n            <li>providing orthodontic services,<\/li>\n            <li>providing services in the field of paediatric dentistry,<\/li>\n            <li>providing periodontal services,<\/li>\n            <li>maintenance of medical records.<\/li>\n          <\/ul>\n        <\/li>\n\n        <li>\n          The detailed scope of tasks, rights and duties of the employees employed in the undertaking of the Treatment Entity shall be regulated by separate internal regulations established by the undertaking which is the Treatment Entity.\n        <\/li>\n\n        <li>\n          The interaction between staff and associates of the healthcare provider should take place in a way that ensures the efficient functioning of the facility, the comprehensiveness, accessibility and sound quality of the healthcare services provided.\n        <\/li>\n\n        <li>\n          The healthcare provider may entrust a designated doctor with the duty to supervise the regularity and quality of the health services provided, the coordination of auxiliary staff, the maintenance of medical records and the processing of the data contained therein.\n        <\/li>\n      <\/ol>\n\n      <h2>\u00a7 11 Medical records and comprehensiveness of services<\/h2>\n      <ol>\n        <li>\n          The health services provided shall be comprehensive. If the Treatment Provider is unable to provide the necessary comprehensiveness within the scope of a given benefit or a specialist consultation is necessary, the person providing the benefit shall inform the patient of the treatment providers who can provide the required assistance.\n        <\/li>\n\n        <li>\n          The healthcare provider shall keep, store and make available medical records in accordance with the rules set out in these regulations and in the legislation on patient rights and medical records.\n        <\/li>\n\n        <li>\n          The healthcare provider shall ensure the protection of the data contained in the medical records.\n        <\/li>\n\n        <li>\n          Medical records are made available in particular on request:\n          <ul>\n            <li>the patient, the patient's legal representative or a person authorised by the patient,<\/li>\n            <li>of the healthcare provider if the records are necessary to ensure continuity of treatment,<\/li>\n            <li>public authorities, bodies of the self-government of the medical profession and national and provincial consultants, to the extent necessary for the performance of their tasks, in particular control and supervision,<\/li>\n            <li>the minister responsible for health, the court, the public prosecutor's office, the medical practitioner and the ombudsman for professional liability, if this is related to the proceedings,<\/li>\n            <li>disability authorities and Disability Assessment Boards in connection with the proceedings,<\/li>\n            <li>insurance company - with the patient's consent.<\/li>\n          <\/ul>\n        <\/li>\n\n        <li>\n          Medical records can be made available:\n          <ul>\n            <li>for inspection at the premises of the Treatment Provider,<\/li>\n            <li>by making extracts, copies or copies,<\/li>\n            <li>by handing over the original against receipt and with the obligation to return it after use, if the authority or body concerned requests that the original records be made available.<\/li>\n          <\/ul>\n        <\/li>\n\n        <li>\n          For providing access to medical records by making an extract, copy or copy, the Treatment Entity shall charge a fee of:\n          <ul>\n            <li>one page of an extract or a copy of medical records - PLN 4,<\/li>\n            <li>one page of copies of medical records - PLN 0.40,<\/li>\n            <li>preparation of an extract, a copy or a copy of the medical records on an electronic data carrier, if the medical records are kept in electronic form - PLN 5.<\/li>\n          <\/ul>\n        <\/li>\n\n        <li>\n          The fees have been set taking into account the actual costs of making copies, extracts, copies and digital copies.\n        <\/li>\n      <\/ol>\n\n      <h2>\u00a7 12 Patient rights<\/h2>\n      <ol>\n        <li>\n          A patient receiving health services provided as part of the activity carried out by the Treatment Entity shall have the rights provided for in the Act on Patients' Rights and Patients' Rights Ombudsman and in separate regulations.\n        <\/li>\n        <li>\n          Information on patients' rights, subject to legal restrictions, is made available to patients at the place where health services are provided in written form, in a place accessible to the public.\n        <\/li>\n        <li>\n          In the case of a patient who is unable to move, the information on the patient's rights shall be made available in such a way that it can be consulted in the patient's room.\n        <\/li>\n      <\/ol>\n\n      <h2>\u00a7 13 Confirmation of death<\/h2>\n      <ol>\n        <li>\n          The patient's death and its causes are stated in the death certificate.\n        <\/li>\n        <li>\n          Issuing a death certificate is the responsibility of the doctor who was the last to provide medical services to the patient in the 30 days before the date of death.\n        <\/li>\n        <li>\n          If it is suspected that the cause of death was a crime, the death certificate shall be issued by the doctor who, on the order of the court or the public prosecutor's office, has carried out an examination or autopsy of the body.\n        <\/li>\n        <li>\n          Where there is no doctor obliged to issue a death certificate or there are other statutory obstacles preventing the examination of the body within the required time, the death certificate shall be issued by the doctor who pronounced the death after the call of the accident or sudden illness.\n        <\/li>\n      <\/ol>\n\n      <h2>\u00a7 14 Management of the Treatment Facility<\/h2>\n      <ol>\n        <li>\n          <strong>AS-Dent Studio Sp. z o.o.<\/strong> with its registered office in Wroc\u0142aw, ul. Krakowska 84\/40, 50-427 Wroc\u0142aw, is managed by the owners, hereinafter referred to as the \u201eOwners\u201d.\n        <\/li>\n        <li>\n          The owners represent the Treatment Entity externally and make decisions regarding the operation of AS-Dent Studio Ltd.\n        <\/li>\n        <li>\n          The duties and powers of the Owners include, in particular:\n          <ul>\n            <li>organising and directing the work of subordinate staff and colleagues,<\/li>\n            <li>ensuring the rational use of working time,<\/li>\n            <li>supervising compliance with the organisation's rules,<\/li>\n            <li>Deciding on the employment, remuneration, punishment and dismissal of employees.<\/li>\n          <\/ul>\n        <\/li>\n      <\/ol>\n\n      <h2>\u00a7 15 Rights and obligations of the patient<\/h2>\n      <ol>\n        <li>\n          While receiving medical services at AS-Dent Studio, the patient is entitled to:\n          <ul>\n            <li>respect for personal dignity and courteous treatment by medical staff,<\/li>\n            <li>medical services provided by appropriately qualified staff in accordance with the requirements of current medical knowledge,<\/li>\n            <li>reliable health information,<\/li>\n            <li>to consent or refuse to give their consent to certain health services, after being informed of the proposed methods of diagnosis and treatment.<\/li>\n          <\/ul>\n        <\/li>\n\n        <li>\n          Patient responsibilities include:\n          <ul>\n            <li>compliance with these organisational rules,<\/li>\n            <li>compliance with the ban on smoking and alcohol consumption on the premises,<\/li>\n            <li>observing the principles of personal hygiene,<\/li>\n            <li>compliance with safety rules and fire regulations,<\/li>\n            <li>follow the instructions of the doctor and medical staff.<\/li>\n          <\/ul>\n        <\/li>\n\n        <li>\n          A patient whose rights have been violated in the course of receiving medical services at AS-Dent Studio has the right to make a verbal or written complaint to the Owners of the facility.\n        <\/li>\n\n        <li>\n          Patients have the right to make comments, requests and complaints to those providing health care services and to address them directly to the Owners of the facility.
